In recent years, antimicrobial resistance has been on the rise and infectious disease specialists other clinicians face several challenges when treating patients with resistant infections. Clinical Laboratory Standards Institute (CLSI) annually publishes guidelines for tests, including revised breakpoints, because of in changes to nomenclature owing advances whole genome sequencing technology. However, many laboratories use historical do not follow breakpoints. Moreover, current breakpoints have limitations. This study evaluated over last 4 from 2018 2021. Here, I describe microbiological clinical background CLSI breakpoint revision evaluate advantages limitations new a review large data. addition, reviewed problems associated each made suggestions how they might be improved, example, increasing or decreasing minimum inhibitory concentration (MIC) zone diameter, deleting adding an S, I, R category, introducing concepts (such as susceptible-dose dependent (SDD)), requesting more evaluation methods. In regions where KPC-producing bacteria are endemic, testing for nonsusceptibility to meropenem may provide improved accuracy in identifying these isolates.
